Work Environment

The work environment is dynamic and community-centric, allowing you to engage directly with patients in their homes. The average workweek consists of approximately 40 hours, with a schedule that typically runs from 9:00 a.m. to 6:00 p.m., including the possibility of evening clinics until 8:00 p.m. Flexible participation in community outreach events or health fairs during weekends may also be required.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse individuals and families, taking into account their unique social and economic circumstances.
• Diagnose and manage both acute and chronic medical conditions, employing evidence-based practices to support preventive care and health promotion strategies.
• Develop and implement educational programs in collaboration with community organizations, enhancing public awareness of health issues and resources.
• Provide culturally sensitive patient care, advocating for their rights and ensuring they have sufficient access to necessary healthcare services.
• Coordinate patient referrals and follow-up care to ensure seamless continuity of care, working closely with specialists and other healthcare providers.
• Accurately maintain detailed medical records in accordance with healthcare regulations and organizational standards.
• Engage in community outreach activities and health fairs, providing essential screenings and health education to raise awareness and foster trust within the community.
• Stay informed about health trends, community needs, and relevant policies to effectively advocate for patient care and community health initiatives.

Patient Population / Client Focus

Your primary focus will be on underserved populations within the Jackson Heights community, many of whom face socio-economic challenges. You will work with diverse client demographics, including individuals and families who may be experiencing barriers to accessing healthcare services. Your goal will be to provide personalized and comprehensive care that not only addresses immediate health concerns but also empowers patients through education and resources.

Qualifications

• A current certification as a Nurse Practitioner is required.
• An active nursing license in the state of New York.
• A Master’s degree in nursing or a related field from an accredited program.

Compensation and Benefits
