Work Environment

This position is located onsite at our hospital facility, where you will work collaboratively with a multidisciplinary team of healthcare providers, including physicians, nurses, and social workers. Your typical work hours will be 9:00 a.m. to 6:00 p.m., with some evening clinics until 8:00 p.m. and occasional weekend community outreach events.

Key Responsibilities

• Conduct thorough health assessments and screenings, identifying the unique medical and psychosocial needs of individuals and families in our community.
• Diagnose and manage both acute and chronic medical conditions, including developing comprehensive care plans focused on preventive care and health promotion.
• Collaborate with local organizations and community stakeholders to create and implement health education programs tailored to community needs.
• Advocate for patients' rights and equitable access to healthcare services, ensuring culturally sensitive care that respects diverse backgrounds.
• Manage patient referrals and follow-up care to guarantee continuity and quality of services provided to individuals.
• Document detailed medical records in compliance with regulatory standards, ensuring accuracy and confidentiality.
• Engage in community outreach initiatives, health fairs, and other public events, raising awareness about health issues and providing crucial health screenings.
• Stay informed about community health trends, evidence-based practices, and national health policies to enhance service delivery.

Patient Population / Client Focus

Your primary focus will be on serving a diverse patient population in Jackson Heights, which is known for its rich cultural mosaic. Many patients may have limited access to healthcare services, necessitating a compassionate approach to meet their specific health needs. You will address a wide range of health conditions and work with populations that include immigrants, individuals with chronic illnesses, and families needing assistance navigating healthcare systems.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in nursing from an accredited institution.
• A minimum of three years of clinical experience in community health or public health settings, with demonstrable hands-on experience in managing a diverse patient population.
• Exceptional interpersonal and communication skills that allow you to engage constructively with patients, families, and colleagues from varied backgrounds.

Compensation and Benefits

The salary for this position ranges from USD 180,000 to 200,000 per year, commensurate with your experience and qualifications.
