Work Environment

This role will be performed onsite at a school setting in Port Washington, greatly interacting with students, families, and staff. The work environment is collaborative, fostering teamwork and communication amongst healthcare professionals. Regular work hours are from 9:00 a.m. to 6:00 p.m., with potential evening clinics extending until 8:00 p.m. Occasional weekend outreach at local health fairs may also be expected, contributing to a workweek that averages around 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings, integrating comprehensive medical histories and physical examinations to inform diagnoses.
• Manage and treat both acute and chronic medical conditions, implementing tailored preventive care strategies that incorporate patient education.
• Collaborate with local organizations and health agencies to design and execute effective community health education programs addressing specific health challenges.
• Provide culturally sensitive care, advocating for patients and ensuring that their rights are upheld within the healthcare system.
• Serve as a liaison for referrals, ensuring smooth transitions to specialized care and maintaining continuity of care for patients.
• Maintain precise and compliant medical records, documenting all patient interactions and treatments thoroughly to align with regulatory standards.
• Engage in community events to raise health awareness, offering essential screenings and resources to promote well-being.
• Stay abreast of current community health trends, best practices in patient care, and relevant healthcare policies to enhance service delivery.

Patient Population / Client Focus

The Nurse Practitioner at Happy Haven Health Center will primarily serve children and families within the school community, addressing their diverse health needs. Our patient population includes individuals from various backgrounds, often facing socioeconomic challenges. The NP will provide not only clinical care but also critical support in navigating health resources, thereby impacting the overall health of the community.

Qualifications

• A current certification as a Nurse Practitioner, demonstrating up-to-date knowledge and competency in nursing practice.
• An active nursing license in the State of New York.
• A Master’s degree in Nursing from an accredited institution, equipping you with advanced clinical skills and knowledge.
• A minimum of three years of clinical experience, ideally within community health or public health environments, showcasing your ability to work effectively in diverse settings.

Compensation and Benefits

This position offers a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
