The Jewish Community Health Center is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our team at the Center for Advanced Cardiac Care (CACC) in Oyster Bay, New York. This full-time, onsite position allows you to work collaboratively with our esteemed Cardiologists and healthcare providers in an environment committed to exceptional patient care and outcomes. Reporting directly to the Chief Nurse Practitioner and the Medical Director within our Cardiology Division, the chosen candidate will hold a pivotal role in delivering inpatient and outpatient cardiology services.

Work Environment

This position is based in our modern, well-equipped hospital facility located in Oyster Bay, where you will enjoy a structured work schedule from Monday to Friday, from 9 AM to 5 PM, eliminating the stresses of on-call duties, nights, and weekends. You will be part of a collaborative environment that encourages professional growth and active participation in ongoing clinical discussions, training, and quality improvement initiatives.

Key Responsibilities

• Collaborate with leading Cardiologists to manage and monitor patients with chronic conditions such as Hypertension and High Cholesterol.
• Conduct thorough assessments and manage patients presenting with acute cardiovascular conditions, including Coronary Artery Disease, Peripheral Artery Disease, Congestive Heart Failure, Valvular Heart Disease (VHD), and Stroke/Transient Ischemic Attack.
• Provide outpatient Electrophysiology services to manage patients with abnormal electrocardiograms and arrhythmias, including those with implanted devices.
• Implement and interpret Stress Testing protocols as required while providing educational support and guidance on lifestyle modifications to patients and their families.
• Document patient histories and perform comprehensive physical examinations, prescribing appropriate medications and treatments as indicated.
• Facilitate referrals to other specialists within the healthcare network when necessary to ensure comprehensive patient care.
• Engage in regular consultations and receive mentorship from on-site Cardiologists and experienced nurse practitioners.

In this role, you will primarily serve a diverse outpatient population with a focus on adult patients suffering from various cardiovascular issues. You will be instrumental in improving their health outcomes through proactive management and education, thus positively impacting their quality of life. The patient demographics reflect the vibrant community of Oyster Bay, allowing for culturally competent care tailored to individual needs.

Qualifications

• Successful completion of a Nurse Practitioner program accredited by a recognized body.
• Possession of a Master’s or Doctoral degree in Nursing.
• Certification from a recognized professional certifying body relevant to the field.
• A current, active NP license in New York State.

Compensation and Benefits

We offer a highly competitive annual salary ranging from USD 120,000 to 160,000, commensurate with experience and qualifications, along with a comprehensive benefits package that includes:
