Full-Time Cardiovascular Nurse Practitioner

Position Overview

The Jewish Community Health Center is actively seeking a dedicated and skilled Nurse Practitioner to join our team in Hempstead, New York. This full-time position focuses on providing exceptional outpatient care in our Center for Advanced Cardiac Care (CACC). Reporting directly to the Chief Nurse Practitioner and collaborating closely with the Medical Director in the Division of Cardiology, the selected candidate will play a vital role in enhancing patient outcomes through comprehensive care and treatment.

Work Environment

This position is located in an Assisted Living Facility in Hempstead, offering an onsite work setting that fosters collaborative relationships among healthcare professionals. Our multidisciplinary team includes Physicians, Nurse Practitioners, Registered Nurses, and administrative support staff, creating a supportive and nurturing environment conducive to professional growth and patient-centered care. Expect a structured work week, Monday through Friday, from 9 AM to 5 PM, with no nights, weekends, or on-call responsibilities.

Key Responsibilities

• Collaborate with a team of Cardiologists to provide comprehensive cardiac care.
• Conduct thorough patient assessments, including detailed medical histories and physical examinations.
• Manage chronic conditions such as Hypertension and High Cholesterol effectively, developing individualized treatment plans.
• Deliver outpatient Electrophysiology services to patients presenting with abnormal electrocardiograms, arrhythmias, and patients with implanted devices.
• Address acute cardiac conditions including Coronary Artery Disease, Congestive Heart Failure, and Stroke/Transient Ischemic Attacks, using evidence-based interventions.
• Perform and interpret Stress Testing to assist in diagnosis and treatment planning.
• Provide education to patients and their families about prevention strategies and the management of heart conditions.
• Refer patients to specialists as needed for comprehensive care.
• Engage in continuous learning and professional development, including participation in case discussions and educational opportunities.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse group of adult and geriatric patients with a range of cardiovascular concerns. Our patient population may include individuals diagnosed with chronic cardiovascular diseases, those requiring follow-up care after hospitalizations, and patients engaged in preventative health measures. The NP's ability to foster a trusting relationship with patients and effectively communicate medical information plays a significant role in improving patient adherence and satisfaction.

Qualifications

• A Bachelor’s degree in nursing followed by completion of an accredited Nurse Practitioner program.
• Master’s or doctoral degree in nursing is preferred.
• Current and valid Nurse Practitioner license for the state of New York.
• Certification in a relevant specialty, preferably cardiology, with demonstrated competency in outpatient care settings.

Preferred Experience

While we welcome qualified applicants from various backgrounds, priority consideration will be given to candidates with direct experience in cardiology or related settings, whether inpatient or outpatient. New graduate Nurse Practitioners with a strong academic foundation and relevant clinical rotations are also encouraged to apply.

Compensation and Benefits

The position offers an attractive salary range of USD 120,000 – 160,000 per year, dependent on experience and qualifications. Additionally, we offer comprehensive benefits that include:
