In this full-time, onsite role, you will engage in delivering high-quality outpatient care under the guidance of the Chief Nurse Practitioner and the CACC Medical Director. Your collaborative efforts will be instrumental in enhancing patient outcomes and supporting the comprehensive cardiology care model.

Work Environment

The successful candidate will work in Kew Gardens, NY, where our state-of-the-art hospital facilities create an ideal environment for delivering compassionate patient care. Within the CACC, you will collaborate closely with a team of physicians, nurse practitioners, registered nurses, and other healthcare professionals, ensuring an integrated approach to cardiology. Our support staff also plays a key role in maintaining a smooth workflow, allowing our NPs to focus on what matters most—patient care and outcomes.

Key Responsibilities

• Conduct thorough patient assessments, including obtaining detailed medical histories and performing comprehensive physical examinations.
• Order, analyze, and interpret diagnostic tests while managing both chronic and acute cardiac conditions.
• Develop and implement individualized, goal-directed care plans in collaboration with your physician counterparts, adjusting strategies as patient needs evolve.
• Thoroughly coordinate hospital admissions and collaborate with inpatient teams to ensure safe and timely discharges, safeguarding continuity of care.
• Educate and counsel patients and their caregivers on disease management, promoting health literacy and self-care practices essential for long-term patient health.
• Document all patient interactions diligently in the electronic medical record (EMR) system, ensuring precise tracking of care and outcomes.
• Participate in ongoing research initiatives, integrating findings into clinical practice to enhance patient care protocols and outcomes.

Patient Population / Client Focus

The NP in this role will focus on a diverse patient population, including individuals with heart failure, pulmonary arterial hypertension, and those who have undergone heart transplant procedures. Your familiarity with outpatient intravenous therapies and mechanical circulatory support will be invaluable, as you assist patients navigating complex treatment plans and living with significant cardiovascular conditions.

Qualifications

• A Master’s degree in Nursing with specialization as a Nurse Practitioner, focusing on adult care.
• Active New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Board certification as a Nurse Practitioner in your area of specialty.
• Current certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), Drug Enforcement Administration (DEA), and National Provider Identifier (NPI).
• Strong computer literacy, including proficiency with electronic medical records systems and common software applications like Microsoft Word, Excel, and PowerPoint.

Preferred Experience

• At least two years of clinical experience in a hospital setting, ideally within cardiology.
• Hands-on experience with managing a patient population undergoing advanced therapies related to cardiovascular health.
• Familiarity with contemporary treatment modalities and interprofessional collaboration within a multidisciplinary healthcare team.

Compensation and Benefits

The compensation for this full-time position ranges from USD 120,000 to 220,000 annually, which will be commensurate with your experience and qualifications.
