Work Environment

The Nurse Practitioner will be part of a close-knit, multidisciplinary team dedicated to home healthcare. The work setting is primarily onsite, visiting patients in their homes across the Hempstead area. Key Responsibilities

• Order and interpret lab tests and diagnostic imaging to provide accurate medical assessments and modify treatment plans based on findings.
• Maintain meticulous patient records using our electronic medical record (EMR) system to ensure compliance and facilitate seamless care transitions.
• Collaborate with regional and local healthcare teams to align with organizational goals, ensuring consistency in care delivery and adherence to clinical guidelines.
• Engage in strategic initiatives focused on value-based care, working towards improving patient outcomes and program efficiency.

• Practice in accordance with ethical standards and the Scope and Standards of Practice set forth by the nurse practitioner profession and board certification.

Patient Population / Client Focus

This role primarily focuses on a diverse patient population, including elderly clients often facing chronic illnesses requiring ongoing management. The Nurse Practitioner will address complex health issues in various settings, including independent living complexes, assisted living facilities, and direct home care. Qualifications

• A Bachelor’s degree in Nursing is required; a Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program is preferred.
• Possession of a current, unrestricted state license to practice as a Nurse Practitioner in New York.
• National certification by either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Authorization to work in the United States without restrictions.
