Work Environment

This full-time position is located onsite, primarily in the home care setting. As a member of our dedicated healthcare team, you will work collaboratively with various healthcare professionals, including primary care physicians, specialists, and support staff. Our administrative team minimizes paperwork, allowing you to focus on patient care and maintain a manageable caseload. Key Responsibilities

• Document and maintain accurate patient records using our electronic medical record (EMR) system to ensure up-to-date and compliant patient data.
• Collaborate closely with clinical leadership and regional operations to achieve both clinical and organizational targets.
• Implement and support strategic initiatives aimed at providing value-based care, improving patient outcomes, and optimizing resource utilization.
• Engage with patients and their families to enhance their satisfaction and foster meaningful relationships that contribute to high retention rates.
• Adhere to ethical practices and the Scope and Standards of Practice as defined by the Nurse Practitioner board certification.
• Conduct systematic evaluations of patient needs, manage chronic and acute conditions, and develop personalized treatment plans.
• Coordinate care effectively with other providers, including specialists and telemedicine services, ensuring holistic treatment for our patients.
• Participate in family conferences, offering advanced care planning and patient education tailored to individual needs.
• Address and implement pharmacy recommendations derived from pharmacist communications regarding patient prescriptions.
• Ensure your presence in assigned facilities during working hours for comprehensive coverage, enhancing resident care outcomes.
• Order, interpret, and manage relevant lab tests and diagnostics, adjusting treatment plans based on findings.

Patient Population / Client Focus

You will primarily engage with senior populations, often dealing with complex medical histories and multifaceted healthcare needs. Our client base includes individuals living in skilled nursing facilities, independent living homes, and those receiving assistance in home settings. As an NP, your commitment to serving these vulnerable groups will play a crucial role in their health management and quality of life.

Qualifications

• Master of Science in Nursing or Doctor of Nursing Practice from an accredited program.
• Active and unrestricted state license to practice as a Nurse Practitioner.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Current DEA license for prescribing medications.
• Eligibility to work in the United States.

Preferred Experience

• Experience in home care settings or with geriatric patients is highly desirable.
• Demonstrated ability to work in flexible environments and adapt to varied scheduling needs.
• Strong commitment to providing care to underserved populations, specifically elderly and low-income individuals.

Compensation and Benefits
