Work Environment

This position primarily operates in a home care setting, where you will deliver medical care directly within patients' residences. Key Responsibilities

• Conduct thorough health assessments, including comprehensive physical exams for patients of all ages, focusing on acute and chronic conditions.
• Order, perform, and interpret diagnostic tests such as lab work and imaging studies to assist in accurate diagnosis and treatment planning.
• Collaborate with physical therapists, social workers, and other essential team members to develop and implement comprehensive, patient-centered care plans aimed at improving health outcomes.
• Prescribe medications judiciously and monitor patient responses to treatments, adjusting plans as necessary to provide the most effective care.
• Provide ongoing health education and counseling to patients and their families about conditions, treatment options, and preventative health measures.
• Engage actively in team meetings and case conferences, sharing insights about patient progress and coordinating efforts across disciplines.
• Maintain meticulous records of patient encounters, documenting all interactions and following up on treatment plans via our electronic health record (EHR) system.
