Family Practice/Primary Care Nurse Practitioner

Total Compensation: $140k-$150k

A mission-driven healthcare organization based near Queens, NY is hiring Advanced Practice Providers to join their patient care team!

This role offers the chance to make a meaningful difference through direct patient care, health education, and community outreach.

What You'll Do:

• Provide comprehensive medical care to pediatric, adolescent, and adult patients
• Take patient histories, make diagnoses, and develop individualized treatment plans
• Perform well-care exams, including gynecological histories and physicals as needed
• Write prescriptions and educate patients on their medications
• Track and follow up on referrals to specialists and outside agencies
• Support immunizations and phlebotomy as needed to keep patient flow smooth
• Promote health education on topics like preventive care, nutrition, chronic disease management, and family planning
• Provide occasional coverage at satellite locations in Brooklyn and Queens
• Collaborate in case conferences and support the development of medical protocols

What We're Looking For:

• Unrestricted NYS PA or NP license
• Minimum of 2 years of clinical experience preferred
• Bilingual (Spanish or other languages) a plus
