Key Responsibilities

• Conduct thorough assessments and accurately diagnose acute and chronic medical conditions, utilizing both clinical expertise and evidence-based practices.
• Develop individualized treatment plans that address specific patient needs, encompassing medication management, lifestyle modifications, and preventive care strategies.
• Prescribe necessary medications and therapies, and arrange for diagnostic tests as required to ensure optimal patient care.
• Educate patients on disease prevention, health maintenance, and potential lifestyle changes that promote overall wellness and health improvement.
• Collaborate with an interdisciplinary team of healthcare professionals to ensure cohesive patient care and timely referrals for specialized services.

Patient Population / Client Focus

This role will serve a diverse population of patients ranging from young children to older adults. As a Nurse Practitioner at Peak Performance Clinic, you will have the privilege of working closely with families and individuals, understanding their unique health journeys, and guiding them through preventative care and chronic disease management.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license in New York.
• A Bachelor’s Degree in Nursing or a related field; a Master’s Degree in Nursing from an accredited program is preferred.
• At least 2 years of clinical experience, preferably within a primary care setting, demonstrating your ability to manage varied medical situations effectively.

Compensation and Benefits

The compensation for this position ranges between $120,000 and $220,000 annually, commensurate with experience and qualifications.
