Position Overview

Gotham Enterprises Ltd is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our Center for Advanced Cardiac Care (CACC) outpatient practice in Westbury, New York. This full-time position offers the opportunity to work onsite in an Assisted Living Facility, focusing on delivering specialized care to patients with cardiac conditions. The successful candidate will collaborate with a multidisciplinary team under the supervision of the Chief Nurse Practitioner and the Medical Director of the Division of Cardiology.

Work Environment

The position is based onsite at our Assisted Living Facility in Westbury, New York, where the ideal candidate will work within a dynamic and collaborative team of healthcare professionals. The NP will have access to advanced medical technologies, electronic medical record systems, and support from various departments, including social work, nutrition, and inpatient care.

Key Responsibilities

• Conduct thorough patient histories and perform comprehensive physical examinations, ensuring accurate assessment and diagnosis of cardiac conditions.
• Order, interpret, and analyze diagnostic tests including EKGs and imaging studies to manage both chronic and acute cardiac problems effectively.
• Develop, implement, and adjust individualized treatment plans while collaborating closely with physicians and other NPs.
• Educate and counsel patients and caregivers on effective disease management and lifestyle modifications outside of hospital settings.
• Coordinate hospital admissions and collaborate with inpatient teams to ensure safe and efficient patient discharges.
• Document all patient interactions meticulously in the electronic medical records (EMR) system, which adheres to our quality standards.
• Engage in ongoing professional development by participating in research protocols and integrating current findings into clinical practice.
• Assist in triaging patient calls and communicating effectively with referring providers to optimize patient care and flow.
• Maintain a responsible and accountable presence in all aspects of Nurse Practitioner practice, ensuring high-quality patient care.
• Perform additional duties as directed by supervisors to support the overall mission of the Center.

Patient Population / Client Focus

This Nurse Practitioner role is tailored for individuals with a passion for cardiac care, focusing on adult patients with a spectrum of cardiovascular conditions. The NP will work with diverse patient populations, including those undergoing treatment for advanced heart failure, pulmonary arterial hypertension, and individuals preparing for heart transplantation. Creating tailored care strategies and providing hands-on education will be crucial in enhancing patient adherence to treatment plans and improving their health outcomes.

Qualifications

• A Master’s degree in Nursing with advanced training as a Nurse Practitioner.
• Active New York State Registered Nurse and Nurse Practitioner licenses.
• Board certification as a Nurse Practitioner.
• Current certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in using electronic medical records programs and standard office software including MS Word, PowerPoint, and Excel.

Preferred Experience

• A minimum of two years of clinical experience as a Nurse Practitioner, preferably within a hospital or outpatient cardiology setting.
• Experience managing patients with chronic heart conditions and complex medication regimens.
• Familiarity with advanced therapies, including outpatient intravenous medication support and mechanical circulatory devices, is a plus.

Compensation and Benefits

The compensation package for this role includes a competitive salary range of $120,000 to $220,000 annually, commensurate with experience and education.
