Position Overview

Gotham Enterprises Ltd is seeking an experienced and compassionate Nurse Practitioner (NP) to join our dynamic team at the Center for Advanced Cardiac Care (CACC) in Saint Albans, New York. As a crucial part of our outpatient cardiology practice, the NP will work under the supervision of the Chief Nurse Practitioner and in alignment with the CACC Medical Director. This full-time role offers the opportunity to deliver personalized patient care in a concierge medicine setting that emphasizes holistic healthcare and house calls.

Work Environment

This onsite position will immerse you in a supportive and collaborative environment where teamwork is paramount. You will interact daily with a range of healthcare professionals, including Physicians, NP colleagues, Registered Nurses, Social Workers, Nutritionists, and other specialists. The active engagement among team members fosters a proactive approach to patient management, emphasizing swift communication and coordinated care strategies, whether in the clinic or during house calls.

Key Responsibilities

As a Nurse Practitioner, your primary responsibilities will include:

• Performing comprehensive assessments by obtaining thorough patient histories and conducting targeted physical examinations to evaluate health status.
• Ordering and interpreting diagnostic tests and utilizing these results to diagnose and manage both chronic and acute cardiac conditions.
• Formulating personalized care plans that incorporate evidence-based interventions, health maintenance strategies, and anticipatory guidance in line with established medical protocols.
• Collaborating with physicians to evaluate treatment plans, making necessary adjustments based on ongoing assessments and patient feedback.
• Offering education and counseling to patients and their caregivers about disease processes, emphasizing effective management techniques outside of clinical settings.
• Coordinating patient admissions to hospitals, ensuring seamless communication with inpatient teams for safe and timely discharges.
• Documenting all patient interactions meticulously within the outpatient electronic medical records (EMR) system to ensure accurate and readily accessible patient information.
• Participating in ongoing clinical research initiatives, applying relevant findings to enhance practice and promote evidence-based care.
• Upholding high standards of safety and effectiveness in nursing practice to foster a therapeutic environment for both patients and staff.
• Fulfill additional responsibilities as assigned, contributing to the overall efficiency and operation of the clinic.

Patient Population / Client Focus

The Nurse Practitioner will primarily focus on patients with a range of cardiac concerns, including but not limited to advanced heart failure, pulmonary arterial hypertension, and recipients of heart transplants. You will be responsible for managing patients undergoing advanced therapies such as intravenous medications and mechanical circulatory support, thus ensuring their complex needs are met effectively and compassionately.

Qualifications

To qualify for this role, candidates must possess the following:

• A Master’s degree in Nursing with specialization as a Nurse Practitioner, particularly in adult care.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• NP Board certification ensuring adherence to high clinical standards.
• Certificates in Basic Cardiac Life Support (BCLS), Advanced Cardiac Life Support (ACLS), DEA registration, and National Provider Identifier (NPI).
• Proficiency in using electronic medical records systems and familiarity with software applications such as MS Word, PowerPoint, and Excel.

Preferred Experience

Ideal candidates will have:

• At least two years of clinical practice experience, preferably in a hospital setting specializing in cardiology.
• Experience managing patients dealing with complex cardiac conditions and therapies that require a high level of clinical judgment.
• A proactive approach to coordinating care across multiple healthcare domains, including involvement with inpatient teams and external specialists.

Compensation and Benefits
