Position Overview

United Medical Clinic is actively seeking a dedicated and skilled Nurse Practitioner to join our team in Bellmore, New York. This full-time, onsite position is essential for delivering high-quality primary care to older adults enrolled in Medicare. Our Nurse Practitioners are empowered to work autonomously while fostering collaborative relationships with physicians and multidisciplinary teams to provide exceptional care tailored to the needs of our patients.

Work Environment

As part of our care team, you will be situated within a collaborative environment comprising physicians, nurse practitioners, physician assistants, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. This team-centric structure allows you to focus on delivering primary care, coordinate patient services, and engage with an extensive network of specialists and hospitals, enhancing the overall patient experience.

Key Responsibilities

• Provide high-quality primary care to a diverse patient population while engaging in routine office visits for both acute and chronic health issues.
• Administer annual wellness visits and perform comprehensive health risk assessments, ensuring thorough documentation that reflects a holistic view of patients’ health.
• Coordinate care across various specialties, testing facilities, and agencies to provide comprehensive support for patients.
• Lead population health initiatives, including implementing screenings and preventive care strategies, such as ensuring eligible patients receive routine breast cancer screenings.
• Assist the care team with phone triage, facilitating timely outreach and follow-up with patients.
• Educate patients and their families about health conditions, treatment options, and care plans to enhance understanding and engagement.
• Participate in promotional and community outreach activities to foster the clinic’s mission within the community.
• Conduct home visits for patients who have difficulty accessing our clinic.
• Report directly to the Medical Director while collaborating closely with operational leadership to drive clinical excellence.

Patient Population / Client Focus

Your role will primarily focus on caring for older adults who are on Medicare. We aim to provide personalized, compassionate care that addresses the unique health challenges faced by this demographic. Our healthcare approach emphasizes preventive care and the establishment of long-term relationships, which helps to build trust and a deeper connection with each patient’s care plan.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from either AANP or ANCC.
• A current, non-probationary state Nurse Practitioner license along with an active DEA license.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine is highly desirable.
• A strong understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary range of $180,000 to $200,000 annually, with variations based on experience and expertise. Our comprehensive benefits package includes:
