Work Environment

This position is based in an onsite assisted living facility, providing you with a dynamic environment where you can deliver direct patient care. Our clinic operates from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also participate in weekend community outreach events, enhancing your connection with patients and the broader community. Expect to work approximately 40 hours per week in this collaborative and supportive setting, where each team member plays a vital role in patient care.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients entering the assisted living facility, ensuring personalized care plans are developed.
• Diagnose and manage a range of acute and chronic medical conditions, providing both preventive care and comprehensive health promotion activities.
• Collaborate with local community organizations to develop and implement engaging health education programs that cater to the needs of the patient population.
• Deliver culturally sensitive care, advocating for patients' rights and ensuring they have access to necessary healthcare and social services.
• Coordinate referrals and follow-up care to guarantee seamless continuity of patient care, conducting regular check-ins to monitor progress.
• Maintain precise and thorough medical records in line with regulatory standards and protocols, documenting patient interactions and care plans effectively.

Qualifications

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with qualifications and experience.
