Full-Time Nurse Practitioner for Innovative Primary Care at United Medical Clinic

Position Overview

The United Medical Clinic is seeking a dedicated and skilled full-time Nurse Practitioner to join our team in Elmont, New York. This role is critical in delivering high-quality, patient-centered primary care to our community's older adult population, mainly those with Medicare coverage. As a Nurse Practitioner, you will work autonomously and collaboratively with a diverse healthcare team to promote patient health and wellness while maintaining a focus on preventive care strategies.

Work Environment

This position is situated in an onsite hospital setting that fosters teamwork and open communication among healthcare professionals. You will be part of a multidisciplinary team, including physicians, Medical Assistants, dedicated medical scribes, registered nurses, care managers, and specialists, working together to coordinate and enhance the clinical care provided to our patients. Key Responsibilities

• Deliver exceptional primary care to a diverse patient demographic through both routine office visits and urgent care situations.
• Conduct Annual Wellness Visits and comprehensive health risk assessments that require thorough documentation and patient engagement to ensure a holistic understanding of their health.
• Facilitate care coordination efforts among various providers, specialists, and testing facilities to ensure seamless patient care.
• Lead population health initiatives, assisting with efforts such as ensuring all eligible female patients receive regular evidence-based breast cancer screenings.
• Engage in proactive phone triage and outreach efforts to follow-up with patients for continuity of care.
• Educate patients about their health conditions, care plans, and available treatment options to empower them in managing their health.
• Participate in promotional and community outreach activities to improve clinic visibility and patient engagement.
• Conduct home visits when necessary to provide care for patients who may have mobility challenges.
• Report directly to the Medical Director while collaborating closely with operational and clinical leadership.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• A valid and active Nurse Practitioner license in the state of New York, free of probationary status.
• Possession of an active DEA license.
• Substantial experience in primary care settings—internal medicine, geriatrics, or family medicine is preferred.

Preferred Experience

• Proficiency in HCC documentation, ICD-10 coding, and comprehensive Health Risk Assessment methodologies.
• Demonstrated enthusiasm for collaborative, cross-functional teamwork.
• Interest or experience in innovative healthcare models focused on population health.
• Bilingual proficiency is considered an asset, enabling improved communication with our diverse patient base.

Compensation and Benefits

We offer a competitive salary ranging from USD 180,000 to 200,000 per year, commensurate with experience and qualifications. Our benefits package includes a quarterly bonus based on quality metrics, six weeks of paid time off that encompasses vacation, sick leave, holidays, and CME, as well as tuition reimbursement options. We provide comprehensive health insurance coverage including vision, dental, and life insurance. Additionally, our 401K plan features an up to 4% company match, vested immediately upon employment.
