In this full-time position, you will play a vital role in delivering exceptional primary care to a diverse patient population, primarily older adults on Medicare. Work Environment

This position is onsite at our clinic, where you will be a critical member of a collaborative care team that includes a physician, physician assistants, medical assistants, and dedicated medical scribes. You will have access to comprehensive support from registered nurses and care managers, ensuring that you can devote your attention to primary care, care coordination, and patient advocacy. Our practice operates within a network that partners with elite specialists and hospitals, allowing us to deliver thorough specialty and acute care for our patients.

Key Responsibilities

• Conduct thorough office visits for both routine and acute health issues, ensuring each patient receives the highest standard of care.
• Deliver annual wellness visits and health risk assessments, focusing on precise and detailed documentation to guide ongoing patient care.
• Engage in care coordination with specialists, testing facilities, and social agencies to facilitate comprehensive patient management.
• Lead population health initiatives, ensuring appropriate screenings and assessments are completed for eligible patients.
• Assist the care team with phone triage and outreach, responding promptly to patient inquiries and needs.
• Educate patients on various health conditions, care plans, and treatment options to promote informed decision-making.
• Participate in promotional activities that enhance the clinic's outreach and engagement within the community.
• Occasionally perform home visits as necessary to assess and support patients in their living environments.
• Perform various other duties as assigned, reporting directly to the Medical Director and collaborating closely with operational leadership.

Patient Population / Client Focus

Your primary focus will be on older adults, many of whom are Medicare recipients. The goal is to keep these patients healthy and engaged in life while navigating their unique health challenges. Your efforts in providing compassionate, patient-centered care will not only help them manage chronic conditions but also enhance their quality of life.

Qualifications

• A Master of Science in Nursing or Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from either AANP or ANCC.
• Possession of an active state Nurse Practitioner license and DEA license, both in good standing.

Preferred Experience

• A proven track record in primary care, particularly in internal medicine, geriatrics, or family medicine.
• Proficiency in HCC documentation, ICD-10 coding, and performing health risk assessments is strongly preferred.
• Experience fostering teamwork and engaging in multidisciplinary collaborations to achieve shared health goals.
• Bilingual capabilities, especially in languages commonly spoken in the Kew Gardens community, will be an asset.

Compensation and Benefits

The annual salary for this position ranges between USD 180,000 and 200,000, commensurate with experience and subject to negotiation. Our comprehensive benefits package includes:
