Work Environment

The selected Nurse Practitioner will work onsite at an Assisted Living Facility in Merrick, NY, collaborating within a multidisciplinary care team. This team comprises physicians, other Nurse Practitioners, Physician Assistants, and support staff, including nursing assistants, dedicated medical scribes, and care managers. Our collaborative environment focuses on primary care, care coordination, and holistic approaches to population health, ensuring that our healthcare providers can prioritize patient care and satisfaction above all.

Key Responsibilities

• Provide exceptional primary care services, conducting thorough office visits for both routine and acute health issues.
• Administer annual wellness visits and comprehensive health risk assessments, ensuring accurate documentation that reflects the nuanced needs of each patient.
• Coordinate care with other healthcare providers, specialists, testing facilities, and support agencies to meet diverse patient needs.
• Lead population health initiatives, ensuring compliance with evidence-based practices, such as routine screenings for eligible patients.
• Assist the care team with phone triage, patient outreach programs, and follow-up communications.
• Educate patients on their health conditions, treatment plans, and preventive measures, thereby empowering them toward better health management.
• Conduct home visits to assess and manage patients who may require additional support in their living environments.
• Engage in promotional activities and outreach initiatives to foster community relationships and awareness of services.
• Collaborate closely with the Medical Director and support operational leadership in achieving clinic goals.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through either AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license and DEA license.
• Proven experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding standards, and health risk assessments.
• A genuine passion for collaborative teamwork and the ability to engage cross-functionally to enhance patient care.
