Work Environment

You will operate in our outpatient office setting, equipped with a collaborative care team that includes a physician, fellow nurse practitioners, physician assistants, medical assistants, and a dedicated medical scribe. The environment emphasizes teamwork and communication, ensuring that each member can contribute to care coordination and patient outcomes effectively. Operating under a Monday through Friday schedule from 9:00 AM to 5:00 PM, this full-time position allows for a balanced work-life integration.

Key Responsibilities

• Deliver exceptional primary care services, addressing both routine and acute health issues through office visits.
• Conduct comprehensive annual wellness visits and health risk assessments, ensuring thorough documentation and understanding of each patient's unique health profile.
• Facilitate care coordination with various providers, specialists, and local agencies to streamline patient health journeys.
• Lead population health initiatives, including ensuring that eligible patients receive timely screenings and preventive care interventions.
• Assist the care team with phone triage and patient outreach to address health concerns promptly.
• Educate patients on their health conditions, proposed care plans, treatments, and healthy lifestyle choices.
• Participate in promotional activities to enhance the clinic's visibility within the community.
• Conduct home visits when necessary to support patients with limited mobility or accessibility.
• Provide additional support as assigned and report directly to the Medical Director while collaborating closely with operational leadership.

Patient Population / Client Focus

You will primarily serve older adults on Medicare, focusing on maintaining their health and enabling them to lead fulfilling lives. Our clinic's innovative model emphasizes quality care over quantity, emphasizing a long-term relationship with each patient that considers their unique health challenges and needs. Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• A valid DEA license for the prescription of medication.
• Prior experience in primary care settings, including internal medicine, geriatrics, or family medicine.

Preferred Experience

• Familiarity with Hierarchical Condition Categories (HCC) documentation and ICD-10 coding.
• Experience conducting Health Risk Assessments (HRAs).
• Proficiency in teamwork and collaborative work environments, with a desire to contribute to evidence-based population health initiatives.
• Bilingual abilities that could enhance patient communication and care integration.

Compensation and Benefits

The competitive salary for this role ranges between USD 180,000 and 200,000 annually, with the potential for a wider range based on individual experience and expertise. Additionally, employees benefit from a comprehensive package that includes:
