Work Environment

This full-time position requires the Nurse Practitioner to work onsite at our Assisted Living Facility in Long Beach, New York. The work schedule is Monday through Friday from 9 AM to 5 PM, allowing for a well-structured work-life balance. In this collaborative environment, you will work closely with a multidisciplinary team to provide comprehensive care, and you will have the opportunity to participate in community outreach activities.

Key Responsibilities

• Perform thorough health assessments and screenings for residents to evaluate their physical and mental health status, ensuring personalized care plans are developed.
• Diagnose, manage, and treat both acute and chronic medical conditions, implementing preventive care strategies that promote long-term health.
• Actively collaborate with local organizations and stakeholders to create and promote health education programs that raise awareness of important health issues.
• Provide culturally aware and sensitive care, advocating for patients' rights and ensuring they have access to necessary healthcare services.
• Coordinate referrals to specialists and ensure effective follow-up care, maintaining continuity in patient care.
• Keep detailed and compliant medical records using electronic health records (EHR) systems, ensuring all patient information is accurate and up-to-date.
• Participate in health fairs and community outreach events to conduct screenings and distribute educational materials on health and wellness.
• Stay informed about emerging community health trends, best practices, and relevant policies to ensure adherence to industry standards.

Patient Population / Client Focus

The Nurse Practitioner will serve a diverse population of elderly residents within the assisted living facility, including individuals with complex medical histories, chronic illnesses, and varying levels of independence. A focus on empathetic, individualized patient care is essential to meet the unique healthcare needs of this clientele, many of whom rely on the advancements in health services to maintain their quality of life.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner (FNP, ANP, or AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Active and unrestricted Nurse Practitioner license in New York State.
• Possession of a current DEA# and NPI# is required for this role.
• Strong clinical assessment and diagnostic abilities.

• Ability to work well independently while also collaborating with various teams.
• Proficiency with electronic health records and other healthcare technologies.

Preferred Experience

Compensation and Benefits

• Competitive annual salary ranging from USD 150,000 to 160,000, commensurate with qualifications and experience.
