Work Environment

You will be working in an Assisted Living Facility, where the atmosphere is welcoming, and the focus is on patient-centered care. Our facility is equipped with cutting-edge medical technologies and resources that facilitate effective treatment plans. We operate during standard daytime hours, with flexibility for occasional evening clinics and participation in community outreach to ensure our services reach those in need. A typical workweek consists of approximately 40 hours, primarily from 9:00 a.m. to 6:00 p.m., with potential evening clinics extending until 8:00 p.m.

Key Responsibilities

• Conduct thorough health assessments, generating individualized care plans based on patient histories and current health conditions.
• Diagnose and manage a range of acute and chronic healthcare issues, implementing preventive measures and health promotion strategies.
• Engage in active collaboration with community organizations to develop and deliver community health education initiatives that inform residents about wellness practices.
• Provide culturally competent care, ensuring that patients’ rights are upheld, and their access to healthcare is prioritized.
• Coordinate referrals and follow-ups to guarantee seamless continuity of care within and outside the facility.
• Accurately maintain patient medical records, ensuring compliance with health regulations while safeguarding patient confidentiality.
• Participate in community health events and health fairs to promote health screenings and awareness initiatives.
• Stay informed about the latest community health trends, evidence-based practices, and relevant healthcare policies impacting patient care.

Patient Population / Client Focus

The primary patient population served includes older adults and individuals with complex healthcare needs who reside within our Assisted Living Facility. These individuals often require ongoing assessment for chronic conditions, as well as support for activities of daily living. Your role will involve providing compassionate care that not only addresses immediate medical needs but also promotes overall well-being through holistic practices.

Qualifications

• Current certification as a Nurse Practitioner in the state of New York.
• An active and unrestricted state nursing license.
• A Master’s degree in nursing from an accredited academic institution.
• A minimum of three years of clinical experience as a Nurse Practitioner, particularly within community health or public health settings, is essential.
• Demonstrated strong interpersonal and communication skills, showcasing your ability to engage effectively with diverse populations.

Compensation and Benefits

This position offers a competitive salary range of $180,000 to $200,000 per year, based on experience and qualifications.
