Our facility prides itself on its multidisciplinary team approach, consisting of 5 board-certified physicians and 5 experienced nurse practitioners. Work Environment

The work environment at Healthy Horizons Clinic is dynamic and fast-paced, with consistent patient interactions being central to our care model. As a Nurse Practitioner working onsite in our hospital-affiliated urgent care facility, you will be part of a supportive team that fosters a positive and collaborative atmosphere. Our typical work schedule consists of 40 hours per week, from Monday to Friday, concluding at 5:00 PM. The facility itself is equipped with state-of-the-art medical technology and comfortable examination rooms to ensure a welcoming experience for both staff and patients.

Key Responsibilities

• Conduct comprehensive physical examinations and maintain detailed patient medical records.
• Order and analyze diagnostic tests, including blood tests, X-rays, and other relevant assessments.
• Diagnose acute and chronic health conditions, formulating appropriate treatment plans for each patient.
• Prescribe medications and treatment regimens, ensuring patients understand their prescriptions and health conditions.
• Educate patients and their families about disease prevention, management strategies, and healthy lifestyle choices.
• Coordinate care with fellow healthcare professionals, ensuring continuity and comprehensive treatment for patients.
• Engage in professional development activities, including research, continuing education, and attending relevant workshops to stay current with healthcare advancements.

Patient Population / Client Focus

As a Nurse Practitioner at Healthy Horizons Clinic, you will work closely with a diverse patient population. Our clinic serves patients of all ages presenting with various urgent care needs, ranging from minor injuries and infections to chronic disease management. We are particularly focused on creating an inclusive environment that welcomes individuals from different backgrounds, especially those who require bilingual support, as Spanish-speaking skills are highly valued in our practice.

Qualifications

• Master’s degree in Nursing and valid Nurse Practitioner license in the State of New York.
• ANP/FNP certification required.
• DEA registration required for prescribing medications.
• Proficiency in Spanish is highly preferred.
• Strong clinical background with experience in acute care, internal medicine, or urgent care settings.
• Commitment to patient-centered care and maintaining ethical standards in medical practice.
• Outstanding communication and interpersonal skills, emphasized by a collaborative team spirit.
• Ability to manage stressful situations and exhibit patience and professionalism in all interactions.

Preferred Experience

• Prior experience in an urgent care or emergency department setting is preferred.
• Expertise in managing both acute and chronic health conditions effectively and efficiently.
• Experience working with electronic health record (EHR) systems to document patient encounters and track treatment plans.

Compensation and Benefits

This full-time Nurse Practitioner role comes with a competitive salary range of $120,000 to $130,000 per year, commensurate with experience, in addition to a robust benefits package. Our benefits include:
