Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, ensuring that each patient receives personalized care tailored to their specific needs.
• Diagnose and manage a wide range of acute and chronic medical conditions, incorporating preventive care strategies and health promotion into patient interactions.
• Collaborate with community organizations and local stakeholders to develop and implement impactful health education programs aimed at improving community health literacy and engagement.
• Advocate for patients’ rights by providing culturally sensitive care and facilitating access to necessary healthcare services.
• Coordinate referrals and follow-up care to ensure seamless continuity of care and support patient journey through the healthcare system.
• Maintain comprehensive and accurate medical records in compliance with healthcare regulations and standards, utilizing electronic health record (EHR) systems effectively.
• Participate in community outreach events and health fairs, engaging with community members to raise awareness about health issues and provide basic screenings.
• Stay informed about community health trends, policy changes, and evidence-based practices to continually enhance the quality of care provided.

Patient Population / Client Focus

As a Nurse Practitioner at NextGen NP Solutions, you will work closely with a diverse patient population that includes individuals from various socio-economic backgrounds. Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of both DEA# and NPI#.
• Strong clinical assessment skills with the ability to diagnose and treat a variety of health issues.
• Exceptional communication and interpersonal skills, enabling effective interaction with diverse populations.

Preferred Experience

• A minimum of 2 years of experience in primary care or family medicine, ideally in community health or public health settings.
• A track record of participation in health education efforts or advocacy for patient care improvements will be considered an asset.
• Proficiency with electronic health records (EHR) systems.

Compensation and Benefits

This position offers an attractive salary range of USD 150,000 to 160,000 annually, reflective of your experience and skills.
