Work Environment

This role is located within our state-of-the-art hospital facility in Rego Park, where you will be part of a dynamic multidisciplinary care team. Your daily collaborators will include physicians, nurse practitioners, physician assistants, medical assistants, and care managers. With the support of a dedicated medical scribe and registered nurses, you will have access to a network of elite specialists and hospitals for any necessary referrals. Our focus is on providing personalized outpatient care while ensuring a streamlined workflow that emphasizes quality over quantity.

Key Responsibilities

• Deliver outstanding primary care to a diverse adult patient base, managing both routine and acute medical issues.
• Conduct comprehensive annual wellness visits, health risk assessments, and document findings thoroughly and accurately, ensuring a holistic understanding of each patient’s health.
• Coordinate care with various healthcare providers, specialists, and testing facilities to promote seamless patient experiences.
• Lead population health initiatives by ensuring that all eligible patients receive necessary screenings and preventive care services.
• Assist in phone triage, proactively reaching out to patients as needed to facilitate their care.
• Educate patients on their health conditions, treatment plans, and options, empowering them to take control of their health journey.
• Participate in community outreach and promotional activities to raise awareness about our services and engage potential patients.
• Conduct home visits for patients unable to come to the clinic, providing continuity of care.
• Collaborate closely with the Medical Director and operational leadership to ensure adherence to clinical best practices and organizational goals.

Patient Population / Client Focus

You will primarily serve adults on Medicare, offering personalized primary care and support. Our approach is focused on preventative care and managing chronic conditions, aimed at enhancing the quality of life and keeping patients out of the hospital. We aim to understand the unique challenges faced by older adults and tailor our services to meet those needs effectively.

Qualifications

• A Master of Science in Nursing, a Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from either AANP or ANCC.
• Possession of a valid, non-probationary Nurse Practitioner license in New York and an active DEA license.
• Solid experience in primary care settings, particularly internal medicine, geriatrics, or family medicine.
• Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A passion for teamwork and a collaborative approach to care delivery.
• Bilingual capabilities are a plus, enhancing communication with our diverse patient base.

Preferred Experience

• Experience implementing innovative care models that prioritize patient-centered approaches and population health metrics.
• Demonstrated competency in handling multi-faceted patient cases with a focus on care coordination.

Compensation and Benefits

The compensation for this role ranges from USD 180,000 to 220,000 annually, with the potential for a wider range based on experience and qualifications. In addition to competitive salary, we offer:
