Work Environment

This full-time position is primarily onsite, providing care directly in patients' homes. Key Responsibilities

• Conduct thorough assessments and diagnoses of patients presenting with acute and chronic health issues that may include cardiovascular disorders, diabetes management, respiratory conditions, and more.
• Develop, implement, and update personalized evidence-based treatment plans in collaboration with other healthcare professionals and the patients’ families.
• Order and interpret diagnostic tests, and prescribe medications and therapeutic interventions tailored to individual patient needs.
• Educate patients and their families on medication adherence, disease management strategies, and post-discharge care to enhance recovery and maintain health.
• Utilize electronic medical record systems to document patient interactions and treatment plans accurately, ensuring compliance with health regulations.
• Participate in case management discussions and interdisciplinary team meetings to align care strategies with the broader goals of patient health.
• Continuously engage in quality improvement initiatives, integrating the latest evidence-based practices into your care approach.

Patient Population / Client Focus

You will primarily work with patients across various age groups experiencing a spectrum of health challenges. This includes those requiring chronic disease management and acute care interventions. Your role will involve regular monitoring of patients’ progress, adjusting care plans as necessary, and engaging with patients to understand and address their specific health concerns.

Qualifications

• Current certification as a Nurse Practitioner and a valid, active state nursing license.
• A Bachelor’s degree in nursing from an accredited institution is required; a Master's degree in nursing is highly preferred.
• A minimum of three years of clinical experience as a Nurse Practitioner, ideally within a home or community care environment.
• Proficient in using electronic medical record (EMR) systems and familiar with diagnostic and treatment technologies.
• Exceptional communication and interpersonal skills to effectively collaborate with patients, families, and other healthcare providers.
• Strong critical thinking and problem-solving abilities to address patient care needs dynamically.

Preferred Experience
