Work Environment

As a Lead Nurse Practitioner, you will primarily work in patients' homes across Jersey City. Key Responsibilities

• Conduct thorough, holistic assessments of patients' acute and chronic medical conditions to develop individualized care plans.
• In collaboration with physicians and healthcare teams, implement evidence-based treatment protocols that address each patient's unique health needs.
• Order, interpret, and follow up on diagnostic tests and communicate results effectively with patients and their families.
• Prescribe medications and therapeutic interventions, utilizing an understanding of pharmacology and potential drug interactions specific to home care.
• Provide education on disease management, medication adherence, and self-care strategies tailored to each patient's circumstances.
• Engage in interdisciplinary meetings to discuss patient progress and modify care plans as needed to optimize outcomes.
• Maintain accurate and comprehensive electronic medical records to document assessments, treatment plans, and patient progress.
• Contribute to the development and implementation of quality improvement initiatives aimed at enhancing patient care and safety.
• Stay current with evolving healthcare regulations, clinical guidelines, and best practices to continually improve service delivery.

Patient Population / Client Focus

This position focuses on a diverse patient population, including individuals with complex chronic health conditions, post-operative patients, and those requiring rehabilitation and supportive care. As our Lead Home Care Nurse Practitioner, you will be challenged to provide care that is culturally sensitive, individualized, and responsive to the unique needs of each patient and family.

Qualifications

• Active certification as a Nurse Practitioner, with a specialization in Family or Adult care preferred.
• A current, valid nursing license to practice in the state of New Jersey.
• A minimum of a Bachelor’s degree in Nursing; a Master’s degree in Nursing is preferred.
• At least four years of clinical experience as a Nurse Practitioner, with a focus on home care or a similar setting.
• Exceptional communication and interpersonal skills, essential for building rapport with patients and collaborating with healthcare teams.

Preferred Experience

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive annual salary ranging from $120,000 to $220,000, commensurate with experience and educational qualifications.
