Work Environment

This is a full-time, onsite position focused entirely on home care, allowing you to work closely with patients in their living environments. Key Responsibilities

• Conduct thorough assessments and accurately diagnose both acute and chronic health conditions in patients within their homes.
• Develop, implement, and monitor individualized treatment plans, while effectively coordinating care with physicians, nurses, and other healthcare professionals.
• Order and interpret diagnostic tests to gain deeper insights into patient health and adjust treatment plans as necessary.
• Prescribe medications and implement therapeutic interventions based on clinical guidelines and patient needs.

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in the state of New Jersey.
