Work Environment

This role will be primarily onsite at a local school, allowing you to engage with students and their families directly. You will work during traditional hours from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach projects, health fairs, and events on weekends may be required, as we strive to expand healthcare access and education throughout Jersey City. Key Responsibilities

• Conduct thorough health assessments and screenings tailored to the unique needs of students, families, and community members.
• Diagnose medical conditions, manage both acute and chronic illnesses, and deliver preventive care aimed at improving long-term health outcomes.
• Collaborate with schools and community organizations to develop and implement educational programs on topics like nutrition, mental health, and preventive care practices.
• Provide culturally competent care, actively advocating for patients’ rights, ensuring they understand their healthcare options, and addressing barriers to access.
• Maintain comprehensive medical records in compliance with state and federal regulations, utilizing EMR systems efficiently.
• Coordinate referrals and follow-up care to promote continuity of services for patients navigating the healthcare system.
• Engage in outreach initiatives and health fairs to raise awareness about health issues pertinent to the community, offering screenings and educational resources.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license are required.

Compensation and Benefits

• We offer a competitive salary range of USD 150,000 to USD 180,000 annually, commensurate with experience and qualifications.
