Work Environment

Your work setting will be primarily onsite, engaging directly with patients in their homes. Your schedule will generally be from 9:00 a.m. to 6:00 p.m., with the potential for occasional evening clinics extending until 8:00 p.m. Participation in community outreach events or health fairs during weekends may also be required, reflecting our commitment to community engagement.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, ensuring a comprehensive understanding of their medical history and current health status.

• Diagnose and manage both acute and chronic health conditions, focusing on preventive care and health promotion strategies that cater to individual needs.

• Collaborate effectively with local organizations and stakeholders to design and implement impactful health education programs that address community-specific concerns.

• Provide culturally sensitive care, advocating for patients' rights and working diligently to ensure equitable access to necessary healthcare services.

• Coordinate referrals and follow-up care to secure continuity of treatment and support for patients transitioning through the healthcare system.

Compensation and Benefits
