Work Environment

This role is primarily onsite at an assisted living facility located in Woodbridge. You will work in a collaborative environment alongside other healthcare professionals, including physicians, nursing staff, and administrative personnel. Our team is dedicated to creating a supportive atmosphere where staff and patients alike feel valued. The work hours generally run from 9:00 a.m. to 6:00 p.m., with occasional evening clinics and weekend outreach events.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents and patients, personalized to their unique health circumstances.
• Diagnose and manage both acute and chronic health conditions, offering preventive care options and engaging in health promotion initiatives.
• Collaborate with community stakeholders and partner organizations to design and implement innovative health education programs aimed at improving community health literacy.
• Deliver culturally competent care while advocating for patient rights and ensuring they have access to necessary healthcare services.
• Coordinate patient referrals and follow-up care plans, ensuring a seamless continuum of care for each individual.
• Maintain a meticulous record of patient medical histories and treatment plans, adhering to all regulatory and compliance standards.
• Engage in health fairs and community outreach events to promote wellness and offer necessary health screenings to the greater community.
• Stay informed on the latest health trends and evidence-based practices pertinent to community and preventive health.

Patient Population / Client Focus

Your primary focus will be on individuals residing in the assisted living facility, as well as other community members who may benefit from your expertise. Qualifications

• A current certification as a Nurse Practitioner is essential for this role.
• Possession of an active nursing license in the state of New Jersey is mandatory.
• A Master’s degree in nursing from an accredited program is required.
• At least three years of clinical experience is crucial, particularly in community health or public health settings.

Compensation and Benefits

The salary for this position ranges from $150,000 to $180,000 per year, commensurate with your experience and qualifications.
