Angelic Harmony Clinic is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team on Staten Island, New York. Work Environment

This position is onsite at our Staten Island facility, where you will work directly with patients in their homes. The work environment promotes teamwork and collaboration, allowing you to engage with multidisciplinary healthcare professionals. Your typical work schedule will extend from 9:00 a.m. to 6:00 p.m., with occasional evening hours until 8:00 p.m. and participation in weekend community outreach events, showcasing our commitment to exceeding community health needs.

Key Responsibilities

• Perform thorough health assessments, including physical examinations, health history reviews, and vital sign checks for patients across diverse backgrounds and conditions.
• Diagnose and manage both acute and chronic health issues while developing individualized care plans that include preventive measures and health education tailored to patient needs.
• Actively participate in the creation and implementation of community health education initiatives, collaborating with local organizations to enhance health literacy and preventive care in the community.
• Foster a culturally sensitive approach to care, ensuring all patients feel respected and understood while advocating for their healthcare rights.
• Facilitate seamless coordination of referrals and follow-up care, ensuring that patients receive continuous support on their healthcare journey.
• Maintain accurate and up-to-date medical documentation in accordance with regulatory standards, employing electronic health record (EHR) systems to ensure efficient information sharing.
• Engage in community outreach activities, such as organizing health fairs and conducting health screenings, as part of our commitment to raising awareness about prevalent health issues.
• Stay informed of the latest trends in community health, evidence-based practices, and relevant health policies to continuously improve care delivery.

Patient Population / Client Focus

This role focuses on serving diverse populations, including low-income families and individuals facing health disparities. You will be tasked with addressing not only their physical health needs but also considering social determinants of health, thus delivering a truly holistic level of care.

Qualifications

• Current Nurse Practitioner certification, with an active nursing license in the state of New York.
• A Master’s Degree in Nursing from an accredited program is required.
• A minimum of three years of clinical experience is essential, particularly in community health or related public health settings.
• Exceptional interpersonal and communication skills are critical, with an ability to build rapport with patients from diverse cultural backgrounds.
• A commitment to social justice and health equity, as well as a passion for advocating for vulnerable populations, is a must.

Preferred Experience

• Experience in community health settings or public health initiatives.
• Proficiency in electronic health records (EHR) and telehealth services to enhance patient care.
• Participatory engagement in community outreach or public health education campaigns is highly valued.

Compensation and Benefits
