Work Environment

This full-time nurse practitioner position is based onsite at our Brooklyn clinic, but it also includes frequent home visits to provide care within the community. The clinic is designed to offer a comfortable and warm atmosphere for both staff and patients, ensuring that everyone feels valued and respected. Your work schedule will typically run from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., as well as participation in community outreach during weekends.

Key Responsibilities

• Conduct comprehensive health assessments and screenings, ensuring a thorough understanding of each patient's medical history and current health conditions.
• Diagnose and manage both acute and chronic medical conditions, and develop preventive care strategies tailored to patient needs.
• Collaborate with local organizations to implement community health education programs, advising on nutrition, exercise, and chronic disease management.
• Advocate for patients' rights and equitable access to services, ensuring culturally sensitive care for our diverse clientele.
• Coordinate referrals and manage follow-up care to maintain continuity for patients with complex health needs.
• Document patient interactions and care plans in detailed and accurate medical records, complying with all regulatory standards.
• Engage in community health events and health fairs to promote wellness and provide essential health screenings to the public.
• Stay informed about community health trends, evidence-based practices, and policies to continually enhance service delivery.

Compensation and Benefits
