Work Environment

In this role, you will work onsite at our Jersey City facility and conduct visits to patients' homes, ensuring flexibility and personalized care. The work setting is highly collaborative, involving teamwork among healthcare professionals to target individual patient needs effectively. Our office hours generally range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., and participation in weekend community outreach events may be expected. Key Responsibilities

• Perform thorough health assessments and screenings for individuals and families, identifying medical and social determinants of health.
• Diagnose and manage a spectrum of acute and chronic medical conditions while emphasizing preventive care strategies and health promotion.
• Work in partnership with local organizations and community stakeholders to design and implement effective health education programs that address prevalent health issues.
• Deliver culturally-sensitive care, safeguarding patients' rights and advocating for enhanced access to healthcare services for all demographics.
• Facilitate seamless patient care by coordinating referrals and follow-up services, ensuring that patients navigate the health system efficiently.
• Maintain meticulous medical records in alignment with regulatory requirements, promoting data integrity and confidentiality.
• Engage actively in community events and health fairs, contributing health screenings and educational resources that empower the local population.
• Stay abreast of emerging community health trends, evidence-based interventions, and changes in healthcare policies that can impact our practice and patient care.

Patient Population / Client Focus

This role places a significant focus on serving a diverse patient population, including underserved and vulnerable communities in Jersey City. Your insights and interventions will address prevalent health concerns such as chronic diseases, preventive health screenings, and general health education, thereby tailoring care to meet the unique needs of each individual. Qualifications

• A Master's degree in Nursing from an accredited institution is required.
• Current Nurse Practitioner certification and a valid New Jersey Nursing license.
• A minimum of 3 to 5 years of clinical experience, with a strong preference for backgrounds in community health or public health settings.
• Exceptional verbal and written communication skills, capable of engaging effectively with a diverse range of patients and community partners.
• Demonstrated commitment to social justice, health equity, and advocacy efforts geared towards supporting marginalized groups.

Preferred Experience

• Experience in a concierge medicine model or house call services is highly desirable.
• Familiarity with electronic health record (EHR) systems to facilitate efficient patient documentation and follow-up.
• Experience in implementing community health initiatives or health education workshops is advantageous.

Compensation and Benefits

We offer a competitive annual salary ranging from $150,000 to $180,000, commensurate with your qualifications and experience.
