Work Environment

This position is primarily onsite at our outpatient office, where you will work closely with a team of healthcare professionals in a collaborative and supportive setting. A typical workweek consists of approximately 40 hours, with a schedule running from 9:00 a.m. to 6:00 p.m., along with occasional evening clinics until 8:00 p.m. Participation in community outreach events and health fairs during weekends may be required, providing opportunities to connect with the community outside of traditional office hours.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, identifying acute and chronic medical conditions.
• Formulate effective treatment plans, offering preventive care and health promotion strategies aimed at improving patient health outcomes.
• Develop and implement community health education programs in collaboration with local organizations, effectively reaching a diverse audience.
• Deliver culturally competent care, advocating for patient rights and accessibility to essential healthcare services.
• Coordinate referrals and follow-up care to ensure seamless continuity of care for your patients.
• Maintain accurate and detailed medical records, complying with all relevant regulatory standards and best practices.
• Engage in community outreach initiatives and health fairs, increasing awareness and access to health screenings.
• Stay informed about current community health trends, evidence-based practices, and develop a working knowledge of relevant healthcare policies.

Patient Population / Client Focus

Your work will predominantly focus on serving vulnerable populations, including low-income individuals and families, ethnic minorities, and other underserved communities in Newark. You'll address their unique healthcare challenges, working to eliminate barriers to access and ensuring that all individuals receive the care they deserve.

Qualifications

• Valid certification as a Nurse Practitioner and an active nursing license in the state of New Jersey.
• A Master's degree in Nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, preferably within community health or public health settings.
• Exceptional interpersonal and communication skills, with the ability to build rapport with diverse populations.
• A strong commitment to social justice, health equity, and advocating for the needs of vulnerable communities.

Preferred Experience

• Prior experience in developing and implementing community health programs.
• Familiarity with electronic health record (EHR) systems and patient management software.
• Experience conducting patient education on chronic disease management and preventive health measures.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to USD 180,000 annually, reflecting the experience and expertise you bring to the role.
