Full-Time Community-Focused Family Nurse Practitioner

Work Environment

This role is set in an onsite work environment where the emphasis is on concierge medicine and house calls. You will have the flexibility to deliver care in various settings, including patients' homes, allowing for a more intimate and comprehensive approach to healthcare. The typical work schedule is from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach events, including health fairs during weekends, may also be required to bolster our community engagement efforts.

Key Responsibilities

• Perform comprehensive health assessments, including physical examinations and screenings for both individuals and families in the community.
• Develop and implement personalized patient care plans, diagnosing and managing acute and chronic health conditions.
• Conduct preventative care actions, including vaccinations and health promotion strategies tailored to diverse populations.
• Collaborate closely with local organizations and stakeholders to create and execute health education programs that address community needs.
• Offer culturally sensitive care while advocating for patients' rights and ensuring access to necessary healthcare services.
• Coordinate referrals and follow-up care, ensuring the continuum of care for all patients.
• Maintain accurate medical records in accordance with healthcare regulations and organizational policies.
• Engage in community outreach activities and health fairs to promote health awareness and provide necessary health screenings.
• Stay informed of community health trends, evidence-based practices, and relevant policies impacting patient care.

Patient Population / Client Focus

In this role, you will primarily serve underserved populations in Paterson and surrounding areas. A strong emphasis will be placed on addressing social determinants of health, ensuring that all patients receive high-quality, equitable care regardless of their background. Qualifications

• A current certification as a Nurse Practitioner.
• An active nursing license issued by the State of New Jersey.
• A Master’s degree in nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, ideally in community health or public health settings.
• Exceptional interpersonal and communication skills, enabling effective interactions with diverse populations.
• A firm commitment to social justice, health equity, and advocacy for vulnerable communities.

Preferred Experience

• Experience in concierge medicine or house call services.

Compensation and Benefits

• A competitive salary ranging from $180,000 to $200,000 annually, commensurate with experience and qualifications.
