We are seeking a dedicated and compassionate Community Health Nurse Practitioner to join our dynamic healthcare team at the Healed Medical Care Clinic in Paterson, New Jersey. This full-time, onsite role focuses on delivering holistic healthcare solutions to individuals and families within the community. Work Environment

The Nurse Practitioner will work in a school setting, providing direct patient care to students and their families. Regular office hours are expected from 9:00 a.m. to 6:00 p.m., with the possibility of extending into evenings until 8:00 p.m. on occasion. Participation in weekend health fairs and outreach events is also required, allowing our team to engage with the community outside of traditional clinic hours.

Key Responsibilities

• Perform thorough health assessments and screenings for diverse community members and families, with a focus on preventive health.
• Diagnose and manage both acute and chronic health conditions, employing evidence-based practices to inform treatment plans.
• Design and implement community health education programs in collaboration with local organizations and stakeholders.
• Provide culturally competent care, ensuring that all patients feel respected and understood, while passionately advocating for their rights to accessible healthcare.
• Facilitate referrals and coordinate follow-up care to deliver continuous support to patients.
• Maintain accurate and detailed medical records that comply with healthcare regulations and standards.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license in New Jersey.
• A Bachelor’s degree in nursing; a Master’s degree is preferred.
• A minimum of 3-5 years of clinical experience, ideally in community health or public health settings.
• Exceptional interpersonal and communication skills, with a proven ability to work effectively within diverse populations.
• A strong commitment to principles of social justice and health equity, aimed at advocating for underserved communities.

Preferred Experience

• Experience in school-based health programs is highly desirable.

Compensation and Benefits

This role offers a competitive salary range of $150,000 – $180,000 per year, commensurate with experience and qualifications.
