Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, including vital sign checks, lab work, and health history evaluations.
• Diagnose and manage both acute and chronic medical conditions, developing customized care plans that incorporate preventive measures and health education.
• Work in collaboration with local community organizations and stakeholders to design and implement impactful health education programs targeting prevalent health issues in the area.
• Provide culturally sensitive care to enhance patient-provider communication and ensure all patients receive fair treatment and advocacy in healthcare access.
• Coordinate referrals and subsequent follow-up care to maintain continuity in patient health journeys, closely monitoring progress and addressing barriers to care.
• Accurately maintain detailed medical records that comply with regulatory standards and facilitate seamless patient care transitions.
• Participate in community outreach events and health fairs to promote health awareness, provide screenings, and support preventative care initiatives.

Qualifications

Compensation and Benefits

We offer a competitive salary range of $150,000 - $180,000 per year, commensurate with experience and qualifications.
