Iron Point Health Spine and Sports Medicine is seeking a dedicated and compassionate Community Health Nurse Practitioner to join our team in Paterson, New Jersey. This full-time, onsite position is designed for an experienced nurse practitioner who is committed to providing holistic healthcare solutions to individuals and families.

Work Environment

The Nurse Practitioner will primarily work onsite, providing care within the community as well as through house calls. The typical work schedule will involve weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Moreover, participation in community outreach events and health fairs on weekends may be required, contributing a vital role in enhancing health awareness in the area. Expect to engage in approximately 40 hours of meaningful work each week, directly impacting the health of our community.

Key Responsibilities

• Conduct comprehensive health assessments, utilizing evidence-based practices to screen for health issues and develop personalized care plans for individuals and families.
• Diagnose and manage both acute and chronic conditions, ensuring a patient-centric approach that encompasses preventive care and health promotion strategies.
• Collaborate with local community organizations and stakeholders to initiate and implement health education campaigns that raise awareness about critical health topics.
• Provide culturally sensitive healthcare, respecting and advocating for the needs and rights of each patient to ensure equitable access to necessary services.
• Coordinate patient referrals and follow-up care, ensuring a seamless transition throughout the healthcare continuum and facilitating necessary support services.
• Maintain meticulous and compliant medical records, applying organizational standards and legal health documentation requirements.
• Engage in community outreach, representing Iron Point Health at local health fairs, where you will provide health screenings and educational resources.
• Stay informed on the latest community health trends and best practices, integrating new knowledge into your care delivery methodologies.

Qualifications

• Current certification as a Nurse Practitioner, with an active and unrestricted state nursing license.
