We are seeking a dedicated and compassionate Family Nurse Practitioner to join our team at Angina Cardiology & Family Health Clinic. In this full-time role, you will provide comprehensive, holistic care to diverse individuals and families within the Paterson, New Jersey community. Work Environment

This position will be primarily onsite, providing you the opportunity to engage directly with patients and collaborate with a multidisciplinary team within our home care setting. You will work in a supportive environment that values collaboration, innovation, and community engagement. The typical work schedule will be from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m., as well as participation in community outreach and health fairs on weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings for both individuals and families, tailoring care to meet their specific needs.
• Diagnose and manage both acute and chronic medical conditions, ensuring a preventative approach to health that promotes wellness and health education.
• Work closely with community organizations and stakeholders to create and implement health education initiatives, helping to raise awareness and improve health literacy locally.
• Provide culturally sensitive care by understanding and respecting the diverse backgrounds of your patient population, advocating for their healthcare rights.
• Coordinate referrals and follow-up care for patients, ensuring they receive comprehensive services that contribute to continuity of care.
• Document patient interactions and maintain comprehensive medical records in line with regulatory standards to facilitate the highest quality of care.
• Engage in community outreach efforts, attending health fairs and local events to provide screenings and promote preventive healthcare.
• Stay informed about local health trends and best practices to continuously improve service delivery and patient outcomes.

Patient Population / Client Focus

Your role will primarily focus on underserved populations within Paterson, where access to healthcare can be limited. You will work with diverse clients, including families, children, and elderly patients, ensuring that each individual receives personalized and culturally competent care tailored to their specific challenges and health needs.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s Degree in Nursing from an accredited institution.
• A strong commitment to social justice and health equity, with an understanding of the challenges faced by vulnerable populations.

Preferred Experience

• A minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
• Demonstrated ability to communicate effectively with a diverse range of patients and organizations.
• Experience in managing complex health conditions within an integrative care model.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with experience and credentials.
