Work Environment

The Nurse Practitioner will work onsite in a supportive and collaborative environment, where the focus is on establishing long-term relationships with patients. Scheduled hours will primarily be from 9:00 a.m. to 6:00 p.m., with the expectation of occasional evening clinics until 8:00 p.m. Participation in weekend community outreach events and health fairs will enhance our presence within the community and provide critical services to those in need. You will work closely with a team of healthcare professionals, ensuring a seamless patient experience.

Key Responsibilities

• Conduct thorough health assessments, including physical exams and screenings, tailored to the needs of individuals and families.
• Diagnose and manage various acute and chronic medical conditions, ensuring preventive care and promoting overall health and wellness.
• Implement health education programs in collaboration with community organizations, focusing on specific health issues prevalent in the Newark area.
• Advocate for patients to ensure equitable access to healthcare services, providing culturally sensitive care to diverse populations.
• Coordinate referrals and manage follow-up care, maintaining continuity of care to enhance patient outcomes.
• Document all patient interactions and pertinent medical history accurately in compliance with healthcare regulations.
• Engage in community outreach initiatives, helping to increase health awareness and facilitate health screenings at local events.
• Stay informed about current health trends, best practices, and local healthcare policies to continuously enhance patient care.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in Nursing from an accredited program.
• A minimum of 3-5 years of clinical experience, particularly with an emphasis on community health or public health settings.
• Exceptional interpersonal and communication skills, enabling effective collaboration across diverse populations and community groups.

Preferred Experience

Compensation and Benefits
