Work Environment

This position is onsite at our hospital facility in Elizabeth, which is equipped with modern medical resources to support patient care. The environment promotes teamwork and collaboration among healthcare professionals, along with opportunities for community outreach during health fairs and wellness events. The average workweek consists of approximately 40 hours, typically from 9:00 a.m. to 6:00 p.m., with some flexibility to accommodate evening clinics until 8:00 p.m. and weekend community events.

Key Responsibilities

• Perform thorough health assessments and screenings for patients, tailoring evaluations to meet the diverse needs of individuals and families.
• Diagnose and manage acute and chronic health conditions while focusing on preventive care, health education, and lifestyle modifications.
• Collaborate actively with community organizations and healthcare stakeholders to design and execute health education initiatives aimed at increasing community awareness and access to essential services.
• Provide culturally competent care, advocating passionately for patient rights and fostering connections to ensure comprehensive healthcare access.
• Coordinate referrals, follow-up care, and resources to maintain continuity of care for all patients.
• Maintain accurate and detailed medical records, adhering strictly to healthcare regulations and organizational policies.
• Engage in community outreach, leading health screenings and educational programs at local health fairs to promote healthy living.
• Stay informed about community health trends, best practices in nursing, and pertinent healthcare policies to effectively serve our patient population.

Qualifications

• Must possess a current Nurse Practitioner certification and active nursing license applicable in the state of New Jersey.

Compensation and Benefits

• The position offers a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
