Work Environment

The position is based onsite at a local school facility in Woodbridge, New Jersey, where you will engage primarily with students and their families. The work setting is vibrant and community-oriented, offering ample opportunities for engagement and outreach. Your typical schedule will run from 9:00 a.m. to 6:00 p.m., with the potential for occasional evening clinics until 8:00 p.m. Participation in community outreach events and health fairs, particularly on weekends, may also be required, providing a platform to directly interact with different segments of the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, tailoring evaluations to meet the unique needs of diverse populations.
• Diagnose, manage, and treat both acute and chronic medical conditions while focusing on preventive care and promoting overall wellness.
• Collaborate closely with community organizations and stakeholders to develop and implement targeted health education programs, utilizing evidence-based practices.
• Provide culturally sensitive care, advocating for patients' rights and their access to necessary healthcare services.
• Facilitate referrals and follow-up care to ensure seamless continuity of healthcare services for patients, collaborating with other healthcare professionals when needed.
• Maintain comprehensive and accurate medical records in accordance with regulatory standards, ensuring compliance with healthcare policies.
• Actively participate in community outreach events and health fairs, working to increase public health awareness and provide necessary health screenings.
• Remain informed on emerging community health trends, relevant policies, and evidence-based practices to enhance service delivery.

Patient Population / Client Focus

Your work will primarily focus on serving students and families within the school community, providing essential healthcare services that address a range of physical, emotional, and mental health issues. You will engage with individuals from diverse cultural and socioeconomic backgrounds, making cultural competence and sensitivity essential to your approach.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in Nursing from an accredited program.
• A minimum of four years of clinical experience, with a preference for experience in community health or public health settings.
• Exemplary interpersonal and communication skills, enabling effective collaboration with patients and community stakeholders.

Preferred Experience

• Demonstrated knowledge of social determinants of health and experience advocating for vulnerable populations.
• Experience developing and implementing health education programs in school or community settings.
• Proficiency in maintaining electronic medical records and familiarity with healthcare compliance regulations.

Compensation and Benefits
