Position Overview

Camp Health Laboratory and Diagnostic Center is seeking a dedicated and skilled Nurse Practitioner to join our team in providing exceptional primary care in an assisted living facility setting. This full-time role will allow you to directly impact the health and well-being of our patients, while collaborating with a multidisciplinary team focused on patient-centered care. The position offers a competitive salary range of $115,000 to $150,000 annually, commensurate with experience.

Work Environment

This position will be based onsite at our Staten Island, New York location within an assisted living facility. Our work environment promotes interdisciplinary collaboration, with a team consisting of a physician, Nurse Practitioners, physician assistants, medical assistants, and support staff including dedicated medical scribes and nursing care managers. Key Responsibilities

As a Nurse Practitioner at Camp Health Laboratory and Diagnostic Center, your core responsibilities will include:

• Providing exceptional primary care by conducting thorough assessments, diagnosing conditions, and developing treatment plans tailored to individual patient needs.
• Conducting office visits to address both routine and acute health issues in a supportive and educational environment.
• Administering annual wellness visits and health risk assessments, emphasizing holistic health management and accurate documentation.
• Coordinating patient care with other providers, specialists, testing facilities, and support agencies to ensure continuity and quality of care.
• Leading population health initiatives, including ensuring compliance with evidence-based screening practices, such as biannual breast cancer screenings.
• Engaging in phone triage and outreach to assist patients and address their needs effectively.
• Educating patients on their health conditions, treatment options, and care plans to foster understanding and encourage adherence.
• Participating in promotional activities and community outreach to raise awareness about available services.
• Conducting home visits as necessary to provide care to those unable to travel to the facility.
• Reporting directly to the Medical Director while collaborating closely with practice operations leadership.

Patient Population / Client Focus

Your primary focus will be on older adults, particularly those on Medicare, who require personalized care to maintain their health and enhance their quality of life. Our innovative care model not only prioritizes the quality of care but also emphasizes the importance of community-based interventions, ensuring that patients receive the support they need in familiar surroundings.

Qualifications

The successful candidate will possess the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or a graduate of an accredited Nurse Practitioner certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active and non-probationary state Nurse Practitioner license.
• An active DEA license for prescribing medications.

Preferred Experience

While all qualified candidates are encouraged to apply, preference will be given to those who have:

• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• An understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and health risk assessments (HRAs).
• A passion for teamwork and collaborative patient care.
• Bilingual proficiency, beneficial for working with diverse patient populations.
