Work Environment

Key Responsibilities

• Conduct thorough health assessments and screenings for residents, identifying their individual healthcare needs and preferences.

Patient Population / Client Focus

You will primarily work with seniors residing in the Assisted Living Facility, addressing a range of medical conditions typical in this population. This includes managing chronic illnesses, coordinating care for mental health issues, and providing preventive services such as vaccinations and screenings. Your role will be crucial in ensuring that residents feel valued, respected, and cared for as they navigate their health challenges.

Qualifications

• A Bachelor’s Degree in Nursing is required, with additional advanced training in a Nurse Practitioner program strongly preferred.
• Must hold a current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP) is mandatory.
• DEA and NPI numbers are required for practice.

Preferred Experience

• 3 years of clinical experience as a Nurse Practitioner, preferably within community health or geriatric settings.

Compensation and Benefits

The position offers a competitive annual salary ranging from USD 150,000 to 160,000, commensurate with experience.
