Work Environment

This position involves working onsite within patients' homes, where you will directly engage with our patient population in a comfortable and familiar setting. As a member of our multidisciplinary care team, you will work closely with physicians, other nurse practitioners, medical assistants, and a medical scribe, all supported by coordination from registered nurses and care managers. This collaborative environment allows for shared expertise and seamless patient care management.

Key Responsibilities

• Deliver exceptional primary care services, including comprehensive assessments and treatment plans for both routine and acute health issues.
• Conduct annual wellness visits and health risk assessments, ensuring thorough and precise documentation that reflects patients' holistic health.
• Engage in effective care coordination, collaborating with specialists, testing facilities, and various agencies to streamline the care process.
• Lead initiatives focused on population health, ensuring compliance with preventive care measures, like breast cancer screenings and chronic disease management.
• Assist the care team with phone triage, patient outreach, and follow-up communications to enhance patient engagement and adherence to treatment protocols.
• Educate patients about their health conditions, treatment options, and lifestyle modifications to empower their personal health management.
• Participate in promotional activities designed to enhance community awareness of services available, contributing positively to the organization’s outreach efforts.
• Conduct home visits when necessary to provide care continuity and personal interaction with patients who may have mobility challenges.
• Perform other duties as assigned, while adhering closely to protocols and reporting to the Medical Director.

Patient Population / Client Focus

Our primary patient demographic includes older adults on Medicare who may face complex health challenges. We aim to provide care that addresses their physical, emotional, and social needs, thereby enhancing their overall quality of life. By focusing on prevention and education, we play a vital role in keeping these patients healthy, active, and engaged in their communities.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP Certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in New York.
• Current DEA license for controlled substances.
• Prior experience in primary care settings, particularly in geriatrics or family medicine, strongly preferred.
• In-depth knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• Strong commitment to teamwork and collaboration within a diverse care team.

Preferred Experience

• Ability to communicate fluently in both English and applicable second languages to serve a diverse patient population.
• Proven track record of implementing evidence-based practices in patient care for better health outcomes.

Compensation and Benefits

The salary range for this position is competitive, ranging from USD 180,000 to 200,000 annually, commensurate with experience and expertise. Beyond competitive pay, we offer a comprehensive benefits package that includes:
