Work Environment

This position is based in an onsite-home care setting where you will interact directly with patients in their homes, understanding their environments and specific healthcare needs. Your work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Weekend participation in community outreach or health fairs may occasionally be required. Expect an average weekly commitment of approximately 40 hours within a dynamic and supportive team atmosphere.

Key Responsibilities

• Perform comprehensive health assessments and screenings for patients in their home environments, ensuring a thorough understanding of their health status, concerns, and goals.
• Diagnose, treat, and manage a range of acute and chronic medical conditions, providing preventive care and promoting health awareness in your patient population.
• Collaborate with various community organizations to design and implement targeted health education programs, aimed at informing patients and families about disease prevention, healthy lifestyles, and available healthcare resources.
• Deliver culturally sensitive care and serve as an advocate for patients’ rights, ensuring they have access to necessary healthcare services and resources.
• Oversee referrals to specialty providers and facilitate follow-up care to maintain the continuity of care for individuals transitioning through the healthcare system.
• Maintain accurate, detailed medical records that comply with healthcare regulations and contribute to the improvement of care quality.
• Engage actively in community outreach events, providing health screenings and engaging with local residents to raise awareness of health issues impacting the community.
• Stay informed of the latest trends in community health, relevant policies, and evidence-based practices to enhance service delivery.

Qualifications

• Must hold an active Nurse Practitioner certification and a current state nursing license.
• A Master’s degree in nursing from an accredited institution is required.

Compensation and Benefits

We offer a competitive salary ranging from $180,000 to $200,000 per year, commensurate with your experience and qualifications.
