Work Environment

This role is primarily onsite, allowing you to directly engage with patients in a warm and inviting clinic environment. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics available until 8:00 p.m. Additionally, participation in community outreach events or health fairs during weekends may be required, ensuring that our services reach those who need it most. You can expect an average workweek totaling approximately 40 hours, fostering a balanced work-life dynamic.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse individuals and families to identify health needs and risks.
• Diagnose, manage, and treat both acute and chronic health conditions while delivering preventive care and health promotion strategies.
• Collaborate with community organizations and stakeholders to conceptualize, develop, and implement effective health education programs tailored to meet community needs.
• Provide culturally responsive care, ensuring that the rights of patients are honored and that they have access to appropriate healthcare services.
• Coordinate referrals and follow-up care to guarantee continuity of treatment and support for all patients.
• Maintain detailed, accurate medical records in compliance with healthcare regulations, ensuring confidentiality and integrity of patient information.
• Actively participate in community outreach events and health fairs, enhancing awareness and access to essential health services.
• Stay informed on community health trends and evidence-based practices to continuously enhance care delivery and patient outcomes.

You will primarily serve a diverse patient population within the Elizabeth community, which includes families, individuals with chronic illnesses, and those seeking preventive care. The emphasis on concierge medicine allows for a personalized approach, fostering long-term relationships with patients and their families. Your role will be pivotal in addressing health disparities and enhancing access to quality care for marginalized groups.

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in the state of New Jersey.
• A Bachelor's degree in nursing or equivalent from an accredited institution.
• A minimum of three years of clinical experience in community health or public health settings is essential.

Preferred Experience

Compensation and Benefits

The salary for this position ranges from $150,000 to $180,000 annually, based on experience and qualifications.
