Work Environment

This position is located onsite at our Assisted Living Facility in Jersey City, New Jersey, an area known for its rich diversity and vibrant community life. You will work in a supportive, collaborative environment alongside other healthcare professionals who share your commitment to patient-centered care. Your work schedule will generally range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Additionally, you may be involved in community outreach events or health fairs during weekends.

Key Responsibilities

• Conduct thorough health assessments and preventive screenings for residents within the Assisted Living Facility, addressing both acute and chronic health issues.
• Diagnose and prescribe treatments for various medical conditions, emphasizing a personalized approach to care and health promotion.
• Collaborate with local community organizations and stakeholders to create and implement impactful health education programs that address community-specific health concerns.
• Provide culturally competent care while actively advocating for patients' access to services and their rights within the healthcare system.
• Coordinate referrals and ensure continuity of care through strong follow-up practices, fostering trust and support within the patient community.
• Maintain accurate and comprehensive medical records in adherence to regulatory standards and organizational policies.
• Participate in community outreach events, providing health screenings and education to raise awareness about important health issues.
• Stay informed about current trends in community health, evidence-based practices, and relevant healthcare policies affecting patient care.

Patient Population / Client Focus

The primary patient population includes elderly residents and adults residing in our Assisted Living Facility, many of whom face complex health issues requiring tailored care strategies. Your ability to build rapport and foster relationships with diverse patients will be key in providing sensitive and effective healthcare solutions. Being situated in a diverse urban area allows for a wide range of patient interactions, enhancing the overall patient healthcare journey.

Qualifications

• Current certification as a Nurse Practitioner.
• An active nursing license in New Jersey.
• A Master’s degree in nursing from an accredited program.
• Strong interpersonal and communication skills, with a proven ability to engage effectively with a range of populations.

Preferred Experience

• Minimum of 3-5 years of clinical experience, particularly within community health or public health settings, is preferred.
• A demonstrated commitment to social justice, health equity, and advocacy for underserved communities.

Compensation and Benefits

This position offers a competitive annual salary in the range of USD 150,000 to 180,000.
