Work Environment

This position is an onsite role within a home care setting, allowing for direct engagement with patients in their own environments. Your work hours will typically fall between 9:00 a.m. and 6:00 p.m., with some flexibility for evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events and health fairs may occur on weekends, enhancing the reach of our health initiatives.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the health of individuals and families while considering their unique circumstances and needs.
• Diagnose a range of acute and chronic medical conditions, prescribe appropriate treatments, and oversee preventive care and health promotion efforts.
• Work collaboratively with local community organizations to develop and implement health education programs tailored to the specific needs of Astoria residents.
• Deliver culturally sensitive care to diverse populations, actively advocating for patient rights and access to essential healthcare services.
• Coordinate patient referrals and follow-up care to ensure a seamless continuum of care and support for every patient.
• Maintain meticulous and compliant medical records that reflect regulatory standards and document various aspects of patient care.
• Engage actively in community outreach efforts, such as health fairs, to promote wellness and provide essential health services to underserved populations.
• Stay informed on emerging community health trends, evidence-based practices, and relevant healthcare policies to enhance service delivery.

Patient Population / Client Focus

In this role, you will primarily serve a diverse patient population within the Astoria area, including families, seniors, and underserved communities. Qualifications

• A current certification as a Nurse Practitioner is required.
• Possession of an active state nursing license.
• A Bachelor's degree in Nursing is essential, with a Master's degree in Nursing preferred.
• Minimum of three years of clinical experience, ideally within community health or public health contexts.
• Excellent interpersonal skills, enabling effective communication and rapport-building with patients from various backgrounds.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with qualifications and experience.
