Work Environment

The Nurse Practitioner will operate primarily in an onsite home care setting, providing care to patients in their own environments. The standard work schedule runs from 9:00 a.m. to 6:00 p.m., with flexibility required for occasional evening clinics extending to 8:00 p.m. Additionally, participation in weekend community outreach events and health fairs will be expected, further reinforcing our commitment to community health.

Key Responsibilities

• Conduct thorough health assessments and screenings, ensuring patients receive personalized care tailored to their unique needs.
• Diagnose and manage a range of acute and chronic medical conditions, employing evidence-based practices to support patient outcomes.
• Implement and facilitate health education initiatives in collaboration with community organizations.
• Provide culturally sensitive care, advocating strongly for patients' rights and access to necessary healthcare services.
• Ensure a seamless continuity of care by coordinating referrals and follow-up appointments, maintaining effective communication with patients and healthcare providers.
• Document all patient interactions meticulously, adhering to regulatory standards while safeguarding patient confidentiality.
• Engage actively in community health events, offering screenings and health education to promote public health awareness.
• Stay abreast of the latest trends in community health, policies, and best practices to continually refine the quality of care provided.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in New York State.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 – 200,000 annually, commensurate with experience and qualifications.
